+ Consistent check-ins to track progress and adjust your plan as needed.

guidance, support, and answers to your non-emergent questions.
**Note: Lab orders and medication prescriptions are available for Arizona residents only.

Initial Consultation & Annual Review:

+ Includes a comprehensive assessment, lab review, medical history review,
and medication discussion.
Prior Authorization Fee (if needed and with FDA approved diagnosis):
+ Price: $70
Medication Treatment Pricing:
+ 12 week hormone program $450 (medications included)
+ 6 week weight loss program $450 (medications included)
+ Please ask about our available discounts for participating in both programs and for military/teacher/first responders.
Please Note:
+ Our practice does not accept insurance.
+ You can use your health insurance for lab testing and medications,
provided you meet the necessary criteria.
+ Consultation and follow-up visit costs may be eligible for payment using
your HSA or Flex card, depending on your individual healthcare plan's
coverage.
